Educational resources for hospitals

The educational resources provided in the toolkit were developed for the NPS DMACS project (2008–09) based on the best available evidence and recommendations from the National Heart Foundation of Australia and the Cardiac Society of Australia and New Zealand, in consultation with the national reference group for the DMACS project (2008–09) and can be used with the DMACS e-DUE audit tool to optimise discharge management of patients with ACS.

  • PowerPoint feedback presentation - facilitates feedback of the evaluated data to local hospital staff. Download a guide to using the presentation.
  • DMACS bookmark – includes a reminder for communication points for patients/carers and the general practitioner (GP). It can be laminated for use at ward stations and for distribution to staff.
  • DMACS discharge summary template - has been designed as a discharge summary for ACS patients and is intended to inform the GP and/or regular cardiologist/general physician about ongoing management of their patient's ACS condition.
  • Discharge management checklist (template for 'active' use) - works as a reminder and as a checklist to ensure each necessary element is addressed prior to the patient being discharged from hospital.
  • DMACS information summary - contains evidence-based information on the four key messages of DMACS. It provides a structure for interactions with hospital staff regarding these messages.


Resources from the National Heart Foundation

The National Heart Foundation (NHF) provides resources, free of charge, to educate patients with coronary heart disease (CHD) and their families about heart attack warning signs, what to do, and why it is important to act quickly. These resources include:
